A copy of the U.S. Department of Energy Isotope and Technical Service Order Form, CA-10-90.COM Rev3 (07/01), is included here for your convenience.

This form is in Adobe(R) Acrobat(R) Reader or Adobe(R) Acrobat(R) Exchange (pdf-fillable) format. It can be filled in on the screen and then printed. If Acrobat Reader is loaded and configured for your web browser, just select the form. When the form has loaded, move the mouse pointer to each field and click. You should see a typing cursor in that field. Mark check boxes by clicking on them. When you are ready to print, use "File...Print" or click on the printer button. If you are using Acrobat, Acrobat Exchange or Acrobat Exchange MLP, you should be able to save the form along with the entered data using the "File...Save As" feature. Please note that Acrobat Reader does not have any data saving capability.
NOTE: Some users have reported that data in the last field does not print. This happens when the cursor remains in the last field in which data was entered. To prevent this, using the mouse, move the cursor out of the last field and click any other place on the form.
