That may be the default for vpopmail, but it's certainly not a normal qmail
default.

No, you'd need to either change your system default delivery instructions or
add a .qmail file for a shell user and ensure you had one or more local
domains (as opposed to virtual).
But this means your problem is entirely a vpopmail one, not a qmail one. The
vpopmail delivery agent you're using is the one that's failing to log the
reasons it's causing your delivery deferrals.
Please contact the vpopmail folks for further help with their buggy product.
