Might it not be better to start with a fresh source of qmail, add in only the patches you know you need, then compile and go? Then you can use the QMAILQUEUE patch to run SpamAssassin, or SpamAssasin+somevirusscanner using a little program like qmail-spamc.c (included in the SA distribution), or use qmail-scanner to do the whole thing for you (with an increase in overhead).
