Unexpected Single user problem

Hello,

We have an 8200 running 4.0d with LSM, suddenly one day we took the
machine down to single user mode and the console displays -


Halting processes
Init : single_user mode
Bc_hostname : cannot find a hostname program

Enter run level (0-9 s or S) : S

It then loops and gives me the same error again until I try something
other than single user.

I remember a reference to this exact problem about a year ago on this
list, but there was never a resolution posted.  Are there any
cause identification and solution to the problem?
