You want to block messages at SMTP time?  The way to do that is to apply the
QMAILQUEUE patch (included in netqmail) and install Bruce Guenter's
qmail-qfilter package.  You then write a tiny filter script that blocks
messages based on whatever policy you like, including attachments, etc.  I
believe that the qmail-qfilter package includes example scripts that would do
exactly what you're asking.
