Hi,

I suggest the razor-tool. It works on my host in combination
with maildrop by using the returncode of razor. This is for
the download via pop3. If you have a permanent connection and
the mailtransfer will be handled via smtp you can use
rblsmtpd. An other way to protect you against spam is the using
of qmail-scanner, razor is a part of it.
